I hereby give permission for my child to participate in the following music activity via electronic communication for such time that this activity is not being offered at a Salvation Army.
I understand that I will be involved in the decision for the format of communication (Zoom) used the time and duration during which these communications will be used, and agree to be present/available during these times.
I also understand that I will be in direct contact with either the Corps Officer or Youth/Music Director and may communicate with them regarding lesson progress, questions, or concerns.
In order to participate, the following must be in place:
Must have written/digital consent from parent/guardian for each participant.
Have a master schedule of meeting times.
When using social media platforms (Zoom, Facebook, Skype), communication with participants be done through adult accounts. *No personal accounts of anyone 17 and younger.
Video Chats/Meetings should be conducted during normal business hours or regular corps program hours.
Leaders/Teachers must have current PTM training.
The Salvation Army’s Policies and Code of Conduct for Use and Social Media and Other Electronic Communications with Minors shall apply here. A copy of the policy is available upon request.
By signing my name below, I am acknowledging that I have read, understand, and accept the guidelines above. I certify that I am the parent/legal guardian of the minor named above and am executing this consent form on his/her behalf.
